Instructions for Online Forms

  1. Please choose the single company form  if you represent a single company. You can include multiple reps’ information on this form, so use this form if you have a large company with multiple reps.
  2. Please choose the single rep form if you are an independent rep. You can include multiple companies’ information on this form, so use this form if you represent multiple companies.
  3. After completing your information, you will be transferred to the payment page to select your booth options and pay for your registration.
  4. Look for a proof a few days after we receive your completed contract. PLEASE respond promptly with your approval or edits so we have the correct info.
  5. If your booth requires power, internet or phone, please print and complete the Utilities Order Form below.

Instructions for Paper Form

  1. Download the paper form.
  2. Complete the first page with your booth requirement and payment info.
  3. Sign the second page.
  4. Fill in your company and individual information.
  5. If you are a major manufacturer, use the form marked “One Company with Multiple Representatives” (If you don’t have all the reps’ info, at least give us their names).
  6. If you are an independent rep with many companies, use the form marked “One Representative with Multiple Companies”.
  7. Look for a proof a few days after we receive your completed contract. PLEASE respond promptly with your approval or edits so we have the correct info.
  8. If your booth requires power, internet or phone, please print and complete the Utilities Order Form below.
